Management of leave applications through system recording and monitoring, including the timely deduction of tardiness in the collected leave balances and facilitating of employees’ discipline for violation of the omnibus rules on leave.

Rehabilitation Leave (this is only applicable for work-related accidents):

Duly filed Leave Application Form (Three (3) Original Copies)

Hospital where the employee is admitted

Police Report (One (1) Original Copy)

Police Station to where the incident happened

Study Leave:

Memorandum of Agreement (Up to 6 months) (One (1) Original Copy)

School or review center where the employee is enrolled

Duly filed Leave Application Form (Three (3) Original Copies)

Paternity Leave:

Duly filed Leave Application Form (Three (3) Original Copies)

Child’s Birth Certificate (One (1) Original Copy)

Client’s file or hospital where the child is born

Maternity Leave:

Certified true copy of approved application for leave (Three (3) Original Copies)

Certified true copy of maternity leave clearance (One (1) Original Copy)

Medical Certificate for Maternity Leave (One (1) Original Copy)

Additional requirements for unused maternity leave (upon assumption before the expiration of the 105-day maternity leave):

Medical Certificate that the employee is physically fit to work

Hospital or attending physician

Certificate of Assumption

Approved Daily Time Record (DTR)
