8. Maternity Leave Benefit Application
Process of granting some measure of financial help and providing a period of rest and recuperation in connection with an employee’s pregnancy. It is a leave of absence granted to female government employees legally entitled thereto in addition to vacation and sick leave.

5. If through a representative, special power of attorney or authorization letter and any valid government issued ID by the representative. Notary Public or and any government institution for the identification card.

1. Client submits all the documentary requirements to the Human Resource Management Officer. The Human Resource Management Officer/ Aide prepare the requested documents for the release of maternity benefits. The Human Resource Management Officer/ Aide reviews the document to be submitted and facilitates the computation and routing of the documents. The office of the Municipal Budget Office for signature and recording. The documents for the maternity leave benefit are submitted to the Municipal Accounting Office for recording and issuance of journal voucher. The documents for is further is submitted to the Municipal Treasurer’s Office for signature of the journal voucher. The maternity leave benefit application documents are submitted to the Mayor’s Office for the signature of the journal voucher. The maternity leave benefit application documents are submitted to the Municipal Treasurer’s Office for issuance of check. And the same is forwarded to the Mayor’s Office for signature of the check.
