3. Present priority card number & Death Certificate Form Receive Death Certificate Form, review the veracity of information and sign. If Deceased died at home: a. Take down medical history of deceased as stated by the informant. b. Review entries. Note: If cause of death is medico legal (i.e. stabbing, vehicular accident, etc.) additional requirements is needed such as: a. Police Report ( if no Autopsy was conducted) None 15 Minutes Municipal Health Officer, Rural Health Physician
