1. ISSUANCE OF MAYOR’S CLEARANCE FOR ELECTRICAL PERMIT
Any person, firm or corporation, including any agency or instrumentality of the government who wants to get a new electrical connection, reconnection, relocation or remodeling of existing electrical installation or for energization of temporary construction facility within the jurisdiction in the Municipality of Manolo Fortich shall apply for an electrical permit.

2. One (1) original and two (2) photocopies Electrical permit form signed and sealed by a Professional Electrical Engineer and the applicant Professional Electrical Engineer
3. Three (3) Photocopies of Updated Tax Declaration Municipal Assessor's Office
4. One (1) Original Copy of Certified True Copy of Land Title and Three (3) photocopies of Original Certificate of Title (OCT)/Transfer Certificate of Title (TCT) Land Registration Authority
5. Three (3) copies of Authorization from the Land Owner (duly notarized/SPA) or Deed of Sale or any proof of ownership if Tax Declaration or TCT if not in the name of the applicant Owner of the Land
6. Three (3) copies of Current Tax receipt or Tax Clearance Municipal Treasurer's Office
7. Three (3) copies of Certification from the Barangay Chairman as to road allowance / Barangay Construction Clearance (attached recent photo of location) Concerned Barangay LGU
8. Two (2) copies of approved Subdivision Plan/ Sketch Plan of the lot described on the attached Certified True Copy of Original Certificate of Title (OCT)/ Transfer Certificate of Title (TCT) Department of Environment and Natural Resources (DENR)/Owner of Land
9. One (1) copy of Decision of Zoning and Evaluation Report Municipal Planning and Development Office (MPDO)
