Death is the end of the life of a person. It is an unavoidable fact already set as part of the cycle of life. Death registrations are needed for many legal purposes hence making it one of the most important documents for the family of the deceased.

CHECKLIST OF REQUIREMENTS: WHERE TO SECURE
1. One (1) photocopy of Birth/Marriage certificate of the decease 1. Philippine Statistics Authority/Local Civil Registry
2. One (1) photocopy of Police Report *If the deceased died resulting to a crime such as murder, reckless imprudence, etc. 2. Municipal Police Station
*If the deceased died in any health institution within the jurisdiction of Manolo Fortich, they are to immediately go to the Municipal Health Office for the evaluation of the Death Certificate before proceeding to the LCR Office for the registration.

3. Proceeds to the embalmer. Signs at the back portion of the certificate. 200.00 (Cemetery fee/Rental Fee for those who will be buried at Tankulan Public Cemetery 10 minutes Embalmer
