8. MATERNITY LEAVE BENEFIT APPLICATION

Process of granting some measure of financial help and providing a period of rest and recuperation in connection with an employees’ pregnancy. It is a leave of absence granted to female government employees legally entitled thereto in addition to vacation and sick leave.

Certified true copy of approved application for leave (Three (3) Original Copies)

Human Resource Management Office

Certified true copy of maternity leave clearance (One (1) Original Copy)

Medical Certificate for Maternity Leave (One (1) Original Copy)

Attending Obstetrician-Gynecologist or hospital where the employee will be admitted

Additional requirements for unused maternity leave (upon assumption before the expiration of the 105-day maternity leave):

Medical Certificate that the employee is physically fit to work

Attending Obstetrician-Gynecologist or hospital where the employee is admitted

Certificate of Assumption

Approved Daily Time Record (DTR)

If through a representative, special power of attorney or authorization letter and any valid government issued ID by the representative.
