2. REQUEST FOR RESCUE VEHICLE TRANSPORT OUTSIDE of AREA OF RESPONSIBILITY

A Frontline service in line with R.A. 10121 Section 12 letter C no. 16 which states to Respond to and Manage the adverse effects of emergencies wherein the LGU provides transportation services to clients who needs to be transferred to a health care institution of higher level located outside of the municipality.

2 . Prepares the following: a. Patient’s documents (e.g. PhilHealth ID) b. Referral/Endorsement from Health Institution of origin c. Medical Staff escort from Health Institution of origin

2.1. Arranges transport with back-up rescue vehicle. (NOTE: THERE SHOULD ALWAYS BE A RESCUE VEHICLE ON STANDBY AT THE MDRRM COMMAND CENTER)
